Air traffic controllers and the pilots flying President Donald Trump’s helicopter sat down to hash out communication breakdowns just one week before a tense near-miss at a major airport. They knew systems were failing. They even agreed on an alternative backup plan.

It didn't work. On August 4, at Ronald Reagan Washington National Airport, that exact failure materialized into a dual takeoff incident that forced a commercial airliner and Marine One into close proximity.

The National Transportation Safety Board laid out these unsettling details in a preliminary report. Everyone involved insists the president was never in actual danger. Even so, the incident highlights a stubborn bureaucratic and technical breakdown that lingers in some of the country's most heavily congested airspace.

Communication lines between aviation officials and military transport have been rocky for years. Following a tragic midair collision near the same airport that claimed 67 lives, the Federal Aviation Administration implemented strict new rules. Under those guidelines, controllers must halt all takeoffs and landings whenever a helicopter crosses paths on a conflicting route.

Yet, rules on paper mean nothing if the people in the tower and the cockpit cannot talk to each other properly.

Tower audio from the day of the incident tells a messy story. The Marine One pilot attempted to transmit the standard "three minutes to life" warning required by a longstanding military-FAA agreement. Seconds later, the controller spotted an oncoming commercial flight and issued a warning. The Marine One pilot caught sight of the traffic, took immediate initiative, and announced a momentary delay. The commercial plane quickly climbed away, avoiding a disaster.

Old Hardware and Forgotten Failures
